1. Consent to Participate I/We, the undersigned parent(s) or legal guardian(s) (“Parents/Guardians”) of the above-named participant (“Participant”), hereby give permission for Participant to take part in all Little Libby Loggers (LLL) sandlot baseball or softball activities, including transportation to and from the field. 2. Acknowledgment of Inherent Risks I/We understand and acknowledge that baseball and softball involve inherent risks of serious injury (including, but not limited to, sprains, fractures, concussions, heat-related illness, and, in rare cases, permanent disability or death). I/We further recognize that this event is intentionally self-organized: players will form teams, call their own plays, and resolve on-field issues with minimal adult supervision, which may increase the unpredictability of play. I/We accept full responsibility for all such risks, whether foreseen or unforeseen. 3. Medical Authorization In the event of injury or illness, I/We authorize LLL representatives to secure, at my/our expense, any necessary medical treatment for Participant. I/We affirm that Participant is in good physical condition and has no undisclosed condition that would restrict safe participation. 4. Release of Liability & Indemnification In consideration of Participant’s opportunity to participate, I/We, for ourselves and on behalf of Participant and our respective heirs, executors, and assigns, waive, release, discharge, and hold harmless LLL, their officers, directors, coaches, volunteers, sponsors, field owners, and any person transporting Participant, from any and all claims or causes of action arising out of or related to Participant’s participation, whether arising from negligence or any other cause. I/We further agree to defend, indemnify, and hold harmless the Released Parties against any claims brought by or on behalf of Participant or any third party. 5. Code of Conduct I/We will encourage Participant to demonstrate good sportsmanship, teamwork, and respect for fellow players. I/We understand that failure to follow basic safety instructions or unsportsmanlike behavior may result in removal from the event without refund. 6. Severability & Governing Law If any portion of this Agreement is found unenforceable, the remaining provisions shall remain in full force. This Agreement shall be governed by the laws of the State of Montana, without regard to conflict-of-law provisions.
